Medical Waiver / Liability Release
All players are required to have a medical waiver on file with the club for each season. Below is the waiver for the 2011 - 2012 season.

The Sun Prairie Soccer Club prefers to have all players register through the online system.
Click on the link below and you can register using a credit card or by check. Please note, if paying by check the Sun Prairie Soccer Club will assess a $35 processing fee for checks returned unpaid by the payee's bank.
We also know that there may be reasons why you cannot use the online system due to your childs birth date or other reasons. If the only way you can register your child is by paper, please use the following registration form.
